Electronic Chain of Custody (COC) and Request for Analysis

Documenting proper and accurate chain of custody records after sample collection is imperative to ensure that data is legally defensible.  Custody should be documented from sample collection through sample analysis at the laboratory.   Each custody transfer should be documented on the chain of custody form from sampler to shipper (if applicable) to laboratory.

The chain of custody also serves as a means for clients to communicate sample information and requested analyses to the laboratory.  At a minimum, the chain of custody form should include the sample ID/name, the collection date and time, requested analyses and information pertaining to how and to whom results should be reported.
